If you do not have a regular doctor, start with a clinic that can become your medical home rather than waiting for the next emergency. Cost, insurance, records and appointment access all matter.

A lot of grown adults don't have a doctor. They go to urgent care when something breaks and otherwise hope for the best. It works until it doesn't: until the blood pressure reading at the pharmacy kiosk is alarming, or the ER says "follow up with your primary care provider in a week" and there isn't one.

Primary care is the person or team that knows the whole story, not just today's symptom. They handle the checkups and screenings, the common problems, the long ones like blood pressure and diabetes, the medicine review, and the referrals, and they're the ones who notice when three specialists aren't talking to each other. It is the cheapest good care there is, and it's the one most often skipped.

Where to look in Baytown

  • The primary care listings below, with hours and a button to call. Ask each office whether it's taking new patients and which insurance it accepts.
  • Houston Methodist Baytown Outpatient Center at 1677 W. Baker Road has a primary care clinic, along with imaging, lab, physical therapy, orthopedics and an occupational health clinic. It opened in February 2019, so a lot of people still think of the hospital on Garth Road as the only door.
  • Houston Methodist Primary Care Group has an office at 2610 N Alexander Dr, Suite 201, open weekdays 8 a.m. to 5 p.m.
  • Kelsey-Seybold Clinic - Baytown, 6300 Garth Rd, lists weekday hours of 8 a.m. to 5 p.m.
  • CenterWell Baytown, 6515 Garth Rd, Suite 150, is a senior primary care practice, open weekdays 8 a.m. to 5 p.m. Ask which Medicare plans it works with before you book.

Choosing one

MedlinePlus suggests thinking about the practical things first: Is the office staff friendly and helpful? Is the office good about returning calls? How easy is it to reach the provider? Then the questions of style: friendly and warm, or more formal; a focus on treating disease, or on wellness and prevention; a provider who invites you to be involved in your own care. Good places to get names are friends, neighbors and relatives, and also your dentist, pharmacist or optometrist. Some practices offer a short meet-and-greet visit before you commit, and it costs nothing to ask.

If you don't have insurance

There are doctors in and around Baytown whose job is to see you anyway. - Harris Health Baytown Health Center, 1602 Garth Road, offers primary care, pediatrics, geriatrics, eye care, mammography, podiatry, psychiatry and counseling, with a lab and pharmacy on site. It's open weekdays 7 a.m. to 5 p.m. (the second Thursday of the month, until noon). The Baytown Clinic Pharmacy in the same building fills prescriptions weekdays 8 a.m. to 5 p.m. See the Harris Health site for how to book. - The Harris Health Financial Assistance Program is for Harris County residents with family income at or below 150% of the federal poverty level. It isn't insurance, and you're asked to apply for Medicaid or CHIP first if you might qualify. Copays run from about $3 for a clinic visit, and the program says services are still provided if you can't pay the copay. Apply online, by mail, or at a Harris Health location. The eligibility line is answered weekdays 8 to 4; find the number on the Harris Health site. - Legacy Community Health, Baytown Garth, 3509 Garth Road, sees adults and children. Its own page lists adult medicine, pediatrics, behavioral health, dental, obstetrics and gynecology, senior primary care, vision, vaccinations and a pharmacy. It's open Monday through Thursday 8 a.m. to 5:30 p.m. and Friday 8 a.m. to 6 p.m., offers a Spanish version of its site, accepts Medicaid, and says its services are open to all regardless of the ability to pay. New patients can book through its online portal. - If cost is the worry, read Medical bills and help paying before you skip the visit. - To find other community health centers, use the federal locator at findahealthcenter.hrsa.gov and search Baytown. Federally funded health centers charge on a sliding scale based on income.

The first visit

Make the appointment when you're well. The first visit is where the doctor learns your baseline, and it's much easier to be known before you're sick.

Bring: - Every medicine you take, in the bottles. That means prescriptions, over-the-counter pills, vitamins, supplements, creams and inhalers. Put them in a bag (more in The medicines in the house). The federal Agency for Healthcare Research and Quality recommends this "brown bag" review for every patient once a year, because the list in a portal is often not what's actually in the cabinet. - Your family history, as best you know it: who had heart disease, diabetes or cancer, and at what age. - Your records, or at least the names of your past doctors so the office can request them. - Three questions, written down, most important first. You won't remember them in the room. Nobody does.

Your records belong to you

Federal privacy law gives you the right to see and get copies of your own medical records, including lab results and imaging, with limited exceptions. A provider can charge a reasonable, cost-based fee. When you leave a doctor, ask for what you'll need: the diagnoses, the medicine list, the vaccines, the important reports. Your history should go with you, not stay in one portal.

On Medicare?

Medicare covers a yearly wellness visit at no cost to you when the provider accepts assignment. Medicare describes it as a conversation-based visit to create a prevention plan, not a routine physical exam. It includes a check for early signs of memory problems, and the provider will offer to talk about advance directives. You can have one every 12 months, but not within 12 months of enrolling in Part B or of your "Welcome to Medicare" visit. If you have a specific problem, book a separate visit for it so the wellness visit stays about prevention. Before you go

  • Ask the office three things on the phone: whether it is taking new patients, which insurance it accepts, and how long the wait is for a first visit.
  • Book a visit when you feel fine, because a new-patient appointment is easier to get before there is a crisis.

  • Ask at check-out how to reach the nurse line after hours and how the office handles refills.
